I just want to thank you for writing a very informative piece on this issue. I just discovered yeaterday my daughter who is 9 months old has a labial frenulum it’s quite low in position. We are in the process of dealing with a Frenelum on my 8 mos old son who has failure the thrive. 2 Ped docs and 1 ENT later all told me that my son did not have it. All told me my son had something neurologically wrong with him. Emailed Dr. Kotlow and he said yes he has it very much. What is the cost of the procedure rough estimate if it is laser? Which is the way I want to go for my son! I hate the thought of putting him under general anesthetic!
I’m taking my 8.5mo old daughter down to Ohio to see Dr. Notestine next week for TT & LT revisions! My sister took my nephew (11mos) a few weeks ago and has been having so much less pain while breastfeeding. (Our local La Leche League group in Lansing, MI has had about a dozen or more people take their children – from weeks old to years old – down to see either Dr. Notestine or Dr. Margolis!) I just took my 2 month old daughter to see Dr. Jesse last week, he was AMAZING! She had both a severe upper lip tie and a severe posterior tongue tie. He was the first doctor that believed me. It has made a huge difference in breastfeeding. We are going back tomorrow after realizing that our 2 year old also has a posterior tongue tie, which explains why she has a thrust. I HIGHLY recommend him!

THANK YOU for this list. I have been searching everywhere for providers who do laser frenectomies. My daughter is 15 months and her ULT is causing all sorts of dental issues but no one in my area (Colorado) performs this surgery. She may also have a TT but I’m not sure. Seems such a simple thing, I don’t know why more don’t do it! Anyway, this is helpful. I wish there was someone nearer to me but hopefully we will be able to get her LT revised soon. Thanks again.

Thank you for creating such a useful blog. It has been a very valuable resource! I wanted to share that I was able to find a very qualified DDS, Dr. Ted Reese, here in Indianapolis, who did an excellent job on my daughter’s (age 2.5) very thick maxillary frenum (classified as a grade IV). The office staff was amazing and the laser frenectomy took less than 3 minutes. Prep was about 20 minutes. He used a little nitrous oxide gas and also gave her juice with a mild sedative mixed in. I felt very comfortable with the whole process and the staff was very experienced and professional. All in all, we were in and out in about an hour. She was back to herself the same day. Hope this helps anyone in the Mid West looking for an option. Kind Regards 🙂

I am so glad I found this website! I am lucky I noticed my daughters severe lip tie very early, when she was 6 weeks old. She is 9 weeks now and will be having the procedure done in two weeks. I have also been fortunate in that we don’t have any issues breastfeeding (at 12 lbs 4 oz she is in the 75th percentile!). However I do want to avoid issues in the future concerning her speech. I have been very nervous until I discovered this story! We will be driving from San Antonio Texas to see Dr. Cole in Fort Worth Texas. It is about a 5 hour drive and we will be driving home immediately after the procedure, should I expect her to be sleepier or fussier than usual on the car ride home?

So glad you found the blog. We did the same you guys are doing: we drove there, got it done and drove back home, about 5 and 1/2 hours for us. No fussiness on the way back experienced by our daughter. You can take some Tylenol with you, but you probably won’t use it. Especially you nursing her, that’s the best way to comfort her if she were a little fussy, but i doubt it. I would love to hear your story and post it among the success stories on the blog, to encourage other Moms! Please, keep us posted! 🙂

We were having a hard time finding someone to perform a laser frenectomy for our 3.5 yr old around the Memphis, TN area, until we went to Desoto ENT in Southaven, MS. Dr Colvin was great and acted like it wasn’t an issue at all to use a laser, in fact that is all he uses! So here is an ENT that performs laser frenectomies.
He is tongue-tied by the way and we had all the common issues when he was a baby with breastfeeding. Poor latch, and I had mastitis twice, and his pediatrician never caught it! Unfortunately, I ended up giving up breastfeeding due to both his and my frustrations. I wish more doctors were aware of this and it was easier to find someone that uses lasers! I will let you know how it turns out!

I have just talked with Desoto ENT Care and they explained that they treat tongue tie with clipping on babies and with laser on older children (frenotomy or frenulotomy), even though they don’t perform Upper-Lip Tie frenectomies with laser. I have just added them to our list for tongue tie procedures with laser. Thanks a lot!

I am 66 years old and just had my tongue-tie lasered away. This was my third frenectomy. I’m told I had my tongue cut at birth. Frenectomy #2 was when I was age 16 and starting orthodontia. I couldn’t touch my tongue to the hard palate of my mouth to hold the molds in place, so I was referred for an in-office frenectomy done with a scalpel.
I saw a new dentist 6 months ago, and he told me I had severe tongue-tie. Apparently the doctor that did the earlier procedure just cut enough enough of the frenum to allow me to touch the roof of my mouth with my tongue, or else scar tissue reattached my tongue over time. None of the many dentists I had seen in the 50 years in between ever said anything about me having a tongue-tie, even though it may well have caused my severe gum recession..
The dentist that just did my procedure is Dr. David Winn, II, and his practice is called Academy Laser Dentistry. The address is 6475 Wall St., Suite 201, Colorado Springs, CO 80918. Phone is 719-260-9000. His website is academylaserdentist.com. I don’t know if he works on babies and children, but he teaches laser procedures to other dentists internationally. I paid $400 out of pocket.
This time I saw an orofacial myofunctional therapist before the frenectomy. She diagnosed me has having a tongue-thrust swallow, some speech difficulties, and also regression of my earlier orthodontia, all due to the tongue-tie. She had me do tongue exercises and stretches for a week before the frenectomy, and then those same exercises 2 weeks post-op. She said to be sure to pull on the tongue after the surgery, holding onto it with gauze pads to get good traction, for 10 seconds, repeated 6-8 times, for 3 cycles a day, to prevent scar tissue from forming and readhering the tongue.
It’s taken 3 weeks for my tongue to heal where the frenum was lasered. I will be working with my myofunctional therapist for about 4 months, doing a set of exercises 3X a day. Each week the exercises get progressively more challenging. My tongue had become flaccid after being tied down at the tip for so long. If the patient is old enough to do a program of tongue exercises, I recommend consulting with an orofacial myofunctional therapist before and after a frenectomy. After the initial consult, many of them will work via Skype.

Yes, some ‘stretching’ exercises of the tongue or lips are necessary in the days following the procedure even on little children; however, the younger the child, the shorter the healing time – about a week on my daughter. It also depends on how severe the frenum was, of course.

I have an unfortunate addendum to add to my frenectomy story. Both my myofunctional therapist and I felt that the dentist did not remove enough of my lingual frenulum, because I have too many challenges in doing the myofunctional exercises, I still have a cleft at my tongue tip, and I can’t stick my tongue out enough. My frenulum still appears as a strong fibrous band of tissue, even though my laser frenectomy procedure appeared to take just as long as YouTube videos I’ve seen of that procedure done on older children.
I was scheduled for a revision in mid-May, 3 1/2 months after the original procedure, because that was the earliest my dentist could schedule it. However, 3 weeks ago, my therapist presented photos of my laser frenectomy procedure to the NE dental hygienist’s conference in a tongue-tie seminar. Some experts in the audience said the dentist hadn’t cut enough. One of the audience members suggested I might have a submucosal frenulum, a class 4 type of tie, and suggested I consult with the ENT she worked for. I saw the ENT last week, and I definitely do have a submucosal frenulum. This is when the thick frenulum is embedded in the underside of the tongue. The ENT said the only procedure that would fix that is a frenuloplasty with a Z-plasty. This is where the frenulum is revised and a vertical incision is made in the underside of the tongue, and then horizontal incisions at the top and bottom, in the shape of a Z. Then the flaps are rotated around and sutured, which when healed frees up the tongue to project forward more. The type of “cutting” procedures I’d had 3 times previously are never sufficient, in his opinion. I will be having the Z-plasty frenuloplasty in 2 months in an outpatient surgery facility under general anesthesia. And then, my tongue should finally be freed up enough. Some dentists/dentists say they can accomplish the same thing with a laser procedure.
So the moral of my story is to be aware of a much harder to diagnosis submucosal frenulum, particularly if a cleft persists and tongue dysfunction still persists. Submucosal frenulums are sometimes called posterior frenulums, which are even still harder to diagnose. However, mine is at the anterior of my tongue, and the thick fibrous band is quite obvious.
This submucosal frenulum could well be related to the thyroglossal duct cyst that I had surgery for at age 2. The thyroid develops in the fetus starting at 4 weeks. The thyroid tissue starts at the back of the tongue in the foramen cecum, and then descends down through a thyroglossal duct until it reaches the lower portion of the neck, which occurs at a fetal age of about 7 weeks. In most people, the tissues of the thyroglossal duct become obliterated, but not for those of us that develop thyroglossal duct cysts. Sometimes, as part of this thyroglossal duct formation, there are fibers invisible to a surgeon’s eye that connect the tongue to the base of the mouth. Dr. Steven Park, MD, in an online podcast, speculates that submucosal tongue ties result from the failure of these fibers to obliterate, because biopsies show the geno-glossus muscle, the muscle anchoring the anterior of the tongue, to be more fibrotic in people with tongue tie. I suspect Dr. Park is right in my case.

Thank you for your invaluable list of doctors. I have felt guilt for over four years at not being able to breast feed my daughter and, more recently, my son. Breastfeeding experts at the hospitals, La Leche League, and our pediatrician all checked for tongue tie but none looked at my kids’ upper lips. When we found the upper lip issue, doctors said it would grow out or that we couldn’t do any procedure until orthodontia to avoid scar tissue impeding the progress of braces…
Thank goodness for the power of parents’ intuition and for not being afraid to be right, even when everyone tells you you’re wrong. We will be heading to LA to see Dr. Jesse soon. I hope that when it is all done, our medical professionals will be open to the concept and will have better answers for future parents!

Yes, breastfeeding experts are only now starting to realize that, when a baby has tongue-tie, in most cases he/she is also lip-tied and the sooner the lip-frenum is cut, the better for the baby/child!
